Insurance Information
Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage. Every effort will be made to closely estimate your co-payment and deductible, but the ultimate responsibility for any unpaid balances is yours. Please understand that insurance is a contract between you and your insurance carrier. If an insurance carrier has not paid within 90 days of billing, any unpaid fees are due and payable in full from you.
Please bring your insurance information with you to the consultation. Once we have a treatment plan, we will call your insurance company to obtain the maximum coverage for your procedure. Following surgery, we will file a claim with your insurance company. Please note that if payment from your insurance company does not cover all charges, you will be responsible for any remaining amount.
If you do not have insurance coverage, payment will be due in full at the time of surgery.
Note that many routine dental services are NOT covered by insurance carriers. Please do not hesitate to ask any questions you may have about dental treatment and how to maximize your dental benefits. We want you to be comfortable in dealing with these matters, and we urge you to consult us if you have any questions regarding our services and/or fees. We will do our best to be sure the appropriate information gets to your insurance company on file.
